Liability Waiver Level One OPL Quantum Healing Training – Release of Liability I, acknowledge that I am voluntarily participating in the Level One OPL Quantum Healing Training (“Training”) facilitated by One Precious Light, Mariah Muntimuri, The Enzo Academy. I understand that the Training involves the use of various tools, including but not limited to crystal shards, tuning forks, pendulums, and other materials as outlined in the Soul Physicians Guide to OPL Quantum Healing Level One Training Manual. Assumption of Risk I recognize that participation in the Training may involve inherent risks, including but not limited to physical injury, emotional distress, or property damage resulting from the use of the tools, techniques, or practices taught. I acknowledge that the Facilitator has provided information regarding the required and optional materials, as well as access to instructional videos and manuals, to guide my participation. I accept full responsibility for obtaining, inspecting, and using these materials safely and in accordance with the provided instructions. Release of Liability In consideration of being permitted to participate in the Training, I hereby release, waive, discharge, and agree not to sue the Facilitator, their affiliates, agents, or representatives from any and all liability for any claims, demands, losses, or damages arising out of or related to my participation in the Training, whether caused by negligence, accident, or otherwise, to the fullest extent permitted by law. This includes, but is not limited to, injuries or damages sustained during the practice sessions, Zoom observation, or submission of the required assessment paper. Health and Safety Acknowledgment I affirm that I am in good physical and mental health to participate in the Training and that I have no known conditions that would prevent me from safely engaging in the activities involved. I agree to discontinue participation and notify the Facilitator immediately if I experience any discomfort, injury, or adverse effects during the Training. Voluntary Participation I understand that my participation in the Training is entirely voluntary, and I may withdraw at any time without penalty. I acknowledge that the Facilitator is not a licensed medical professional, and the Training is not intended to diagnose, treat, or cure any medical condition. Any decision to apply the techniques or practices learned is made at my own discretion and risk. By signing below, I confirm that I have read and understood this Release of Liability, and I agree to its terms knowingly and voluntarily. Confidentiality Agreement Level One OPL Quantum Healing Training – Confidentiality Agreement This Confidentiality Agreement (“Agreement”) is entered into between One Precious Light, Mariah Muntimuri, The Enzo Academy and "Participant" in connection with the Participant’s enrollment in the Level One OPL Quantum Healing Training (“Training”). Purpose The Training involves the exchange of proprietary information, including but not limited to the Soul Physicians Guide to OPL Quantum Healing Level One Training Manual, instructional videos (e.g., Flower of Life Certification Video and Level One Celestial Purification Video), energy maps, and personal feedback provided during practice sessions, Zoom observations, and analysis sheet reviews. The purpose of this Agreement is to protect the confidentiality of this information and ensure its use solely for the intended educational purposes. Confidential Information The Participant acknowledges that all materials provided by the Facilitator, including written manuals, video content, passwords (e.g., “Lotus,” “Celestial4444”), and verbal or written feedback, are considered confidential and proprietary. Additionally, any personal insights, experiences, or analysis shared by the Participant during the Training (e.g., through analysis sheets or Zoom calls) will be treated as confidential by the Facilitator. Obligations of the Participant The Participant agrees to: Use the confidential information solely for the purpose of completing the Training and not for any commercial, personal, or unauthorized purposes. Refrain from disclosing, reproducing, or distributing any confidential materials, including manuals, videos, passwords, or energy maps, to any third party without prior written consent from the Facilitator. Protect the confidentiality of any personal information shared by the Facilitator or observed during the Training process. Obligations of the Facilitator The Facilitator agrees to: Maintain the confidentiality of any personal information, analysis sheets, or observations shared by the Participant during the Training, except as required by law or with the Participant’s explicit consent. Use such information solely for the purpose of providing guidance, feedback, and assessment as part of the Training. Exceptions This Agreement does not apply to information that: (1) is publicly available through no fault of either party; (2) was already known to the Participant prior to the Training, as evidenced by written records; or (3) is required to be disclosed by law, provided the disclosing party notifies the other party promptly. Duration This Agreement shall remain in effect during the Training and for a period of 5 years, following its completion, unless otherwise agreed in writing by both parties. Acknowledgment By signing below, the Participant and Facilitator affirm that they have read, understood, and agree to abide by the terms of this Confidentiality Agreement. Both parties recognize that any breach of this Agreement may result in termination of the Participant’s involvement in the Training and/or legal action, as permitted by law.
