Release of Liability Waiver In exchange of the fee paid by you, Mariah Muntimuri and the practitioners and teachers of OPL Quantum Healing and The Enzo Academy, are providing a service assisting you in mind, body, spirit integration through the use of modalities that may include, but not limited to: quantum healing treatment, sound baths and healing, guided meditations and imagery, chakra balancing, energy purification, private consultations, yoga, martial arts, wellness coaching, and energy work. You, the participant, hereby agree to assume all risks associated with any outcome resulting from your participation in The Enzo Academy or OPL Quantum Healing programs or classes or private consultation with their practitioners. Side effects may include, but not limited to: emotional release, lightheadedness, mild fatigue, physical tingling, and change in body temperature, purging, other forms of transmutation and integration of the session work or energy adjustments. All information and mental or physical activity or guidance and direction are provided during programs, classes, or private consultations to enhance the well-being of you, the participant. It is my responsibility, as participant, to inform the practitioner if any discomfort should arise. It is my responsibility, as participant, to notify the practitioner ahead of treatment of any preexisting medical conditions, injuries.. By signing this Waiver and Release of liability you agree that neither Mariah Muntimuri, The Enzo Academy, OPL Quantum Healing and/or any teachers or practitioners shall be responsible for any negative consequence of any kind which is claimed to result from: 1. your participation in any of these programs, activities or consultations; or 2. from the use of any information learned or obtained during these programs, activities or consultations, including, but not limited to, reiki treatment, guided meditations and imagery, sound baths and healing, 3. chakra healing, energy work, yoga, martial arts, wellness coaching, or private consultation provided during The Enzo Academy and OPL Quantum Healing programs, classes, or private consultations. By signing this Waiver and Release you further agree not to assert any legal claims of any kind in any form against Mariah Muntimuri, The Enzo Academy, or OPL Quantum Healing or any affiliated practitioners or teachers on any asserted negative consequence resulting from your participation in these programs, classes or private consultations. By signing this Waiver and Release you acknowledge and agree that Mariah Muntimuri, The Enzo Academy, OPL Quantum Healing, and all practitioners and teachers, disclaim all liability for any injuries or negative consequence claimed by you to be the result of your participation in these programs, classes, or private consultations. By signing this Waiver and Release for a class, event, or workshop you grant permission to the rights of your image, likeness and sound of your voice as recorded on audio or video for promotional use by The Enzo Academy and OPL Quantum Healing without payment or any other consideration, Photographic or video recordings of you may be electronically displayed via the internet or other promotional materials. Private one-on-one sessions or information exchange will require explicit consent on your behalf before sharing. Confidentiality Agreement The Enzo Academy Member agrees to use reasonable efforts to maintain the confidentiality of all the included information, including but not limited to video's, OPL Quantum Healing Sessions, Soundscapes, Workshops, Classes, Mediations, and agrees not to use any Confidential Information for its own benefit or that of a third party unless authorized in advance in writing by Mariah Muntimuri, The Enzo Academy, or OPL Quantum Healing. The information included in The Enzo Academy is Copyrighted by Mariah Muntimuri and is not to be copied, sold, or reproduced in any capacity unless authorized in advance in writing. I acknowledge that there are no refunds for the access to the intellectual property included in The Enzo Academy and that I am free to cancel my enrollment at any time through 'My Account" dashboard.
